ASSOCIATION WISH LIST
The Hospice and Palliative Care Association of Michigan is a non-profit, 501 C 3 organization that provides education, training, resources and advocacy for hospice and palliative care professionals. Because of continued reimbursement cuts to hospice reimbursement, many of our Members are unable to pay for additional training fees, purchase resources and best practice tools. Because we believe that everyone in Michigan deserves the very best in end-of-life care, HPCAM strives to raise additional funds via donations, grants and other means to ensure that those working in hospice and palliative care have access to low cost, high quality education and services. Below are some of the key Funds set up within HPCAM to address these needs.
HOSPICE VETERANS PARTNERSHIP
HPCAM, in partnership with the Michigan VA, coordinates a group of providers who are committed to improving end-of-life care for our veterans of the military and their families. A series of web based trainings is currently under development to use by all hospice providers in Michigan. Other goals of this group include a website specific to caring for vets, development of best practices in spiritual care and bereavement services, tools for developing a Vet to Vet volunteer program, among others.
HONORING HEALTHCARE CHOICES – MI
The Association has been the administrative home for a statewide collaborative focused on expanding advance care planning and particularly the development and piloting of a Michigan version of the POLST form (MI-POST). Funds contributed to this work will help the Association continue to provide training for facilitators on use of the form, education for EMS personnel, conferences and trainings for healthcare professionals and advocacy for changes to Michigan law that recognize advance care planning as a critical element of healthcare delivery.
Contributions to the HPCAM Education Fund support training and educational events for hospice and palliative care professionals. These events are held in person, via the web and day-long seminars. HPCAM maintains a low cost commitment to Members and therefore uses funds from this area to offset costs. The Association offers access to the latest information, technology and tools to assist Members in improving the end-of-life care in Michigan.
SONDRA SEELEY SCHOLARSHIP FUND
Sondra was a tremendous hospice advocate in Michigan. She was passionate about hospice care and upon her passing, the Association created this fund to honor that passion. Contributions to this fund provide scholarships for deserving hospice aides to further their development. Hospice aides provide some of the most important, intimate care for hospice patients and their loved ones. They are often the unsung heros of hospice.
CONSUMER RESOURCE FUND
Developing best practice resources and tools for both hospice and palliative care professionals and for consumers is part of the role of the Association. Support for ongoing publication of the Consumer’s Guide to Hospice Care and the Physicians Guide to Hospice Care are needed at this time.
